Where it states on PF095 'Section D - Employer Organisation / Licensing Authority details' applicants are required to put the following:

Where it states on PF095 'Section C - Purpose of check' applicants are required to use one or more from the above list 1–5. For example, if you are applying for Drivers Authorisation / Bulk Driver and Security Sensitive Substances you would write in:
